AFFIDAVIT OF AUTHORITY FOR VEHICLE
PURSUANT TO GENERAL POWER OF ATTORNEY

Instructions

This affidavit may be presented to a county treasurer or the Department of Transportation as a supplement to a general power of attorney (POA) to conform with Iowa Code 321.49 and Administrative Rule 761-400. This affidavit is to be completed as part of the transaction and must accompany a notarized POA. A separate affidavit is required for each vehicle being transacted under the general POA. Alternately, DOT Form 411021 Power of Attorney Authorization or a vehicle-specific POA may be submitted in lieu of both the general POA and this affidavit.

I have been appointed as agent/attorney-in-fact by

whereby I am authorized in the principal’s name to sign documents pertaining to titling and registering this vehicle as of this date, e.g. make and execute application for certificate of title, assign title and act on all of the title and interest to the described vehicle herein, to make application for replacement title, or application for notation of security interest covering the described vehicle herein. All information provided on this affidavit is true and correct and will warrant and defend the title of any person to the described vehicle insofar as the claim rests upon the title issued by virtue of this affidavit. I will defend, indemnify, and hold harmless the county treasurer and Department of Transportation from any claims or damages that might arise from the actions sworn to herein.
