DESK AUDIT FORM

Contractors shall provide the following information for all employees who worked on any Iowa DOT let project(s) during the past twelve (12) months.

The Company shall submit a list of all employment referral sources (including unions and/or locals) by name. This list must include the following information: name of the recruitment source; contact person; method of contact (e-mail or telephone); date(s) of contact. The Company shall ensure that its EEO/AA Policy reflects the actual recruitment activity the Company engages in.

Please provide a spreadsheet(s) listing all applicants for non-Managerial job classes by name, race, sex, and date of application, job classification applied for, recruitment and hiring source(s), and the outcome of each application.

Please provide a spreadsheet(s) listing all newly hired employee(s), by name, race, sex, job classification, date of hire, and specific job referral source(s).

Please provide a list of all employees assigned to any Federal or Non-Federal Aid project let by the Iowa DOT during the previous twelve (12) months. This list shall include: employee name, race, sex, job classification, start date of employment, date of last job classification change, hours worked, current rate of pay.

Please provide a list of all employees and trainees assigned to any Federal or Non-Federal Aid project let by the Iowa DOT during the previous twelve (12) months. This list shall include: name, race, sex; whether promoted, demoted, transferred, or terminated. This list must also include the dates of these actions. If the employee was promoted or demoted, include the previous and new job classifications as well as wage rates corresponding to both classifications.

Please complete the chart below including all employees who worked on Iowa DOT let projects during the previous twelve (12) months.

If Yes, provide agenda(s), minutes, and sign-in sheet(s).

If Yes, provide documentation, i.e., Employee Handbook, or any other proof of documentation.

If Yes, provide checklist with dates and signatures.

Please complete enclosed PR-1391 Form Using Data From Your Company?s Peak Employment Week During The Previous Twelve (12) Months, on any Federal or Non-Federal aid project let by the Iowa DOT. In the alternative, you may provide a copy of the 1391 form you most recently submitted to the Civil Rights team.
